Banner Home Health is a four-star home care agency with a track record for quality and excellence, and is the largest skilled home care agency in Phoenix. Our structure provides Home Health Nurses with multiple support avenues, including a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available daily for documentation questions), a QI team, telehealth resources, a clinical manager, and in-office triage support. This setup helps manage caseloads effectively and ensures nurses do not feel "alone" in the field. We recognize that nurse case managers are key to our success. As a Wound Care Registered Nurse with our Home Health team, you will manage wounds and supplies enterprise-wide, monitor wound status, and provide both in-person and virtual field support. You will also teach a monthly class for new staff. We offer mileage reimbursement, a company cell phone, and a tablet for documentation in our electronic medical record system (Home Care Home Base).

This is a HYBRID role requiring some in-office time at our Mesa Corporate or Sun City offices, with occasional visits to our Payson, Tucson, and Greeley, CO offices. Monday - Friday 8am - 5pm; optional 4-10's.

POSITION SUMMARY

This role applies specialized knowledge to assess, develop, implement, and evaluate patient care plans for a specific patient population. It involves integrating knowledge into patient and staff education, consultation, and coaching to enhance care quality.

CORE FUNCTIONS

1. Manage and influence patient outcomes and clinical care quality through specialized knowledge, collaborating with staff to improve care and control costs.
2. Provide specialty-focused nursing consultation on complex cases, evaluating needs, recommending treatments, and assessing outcomes.
3. Serve as a clinical resource for patients, families, and staff to improve care quality.
4. Identify staff development opportunities through training and education.
5. Stay current with specialty developments and incorporate findings into practice, policies, and procedures.
6. Engage with staff across departments, physicians, patients, families, and external contacts to coordinate care, provide spiritual support, and synthesize data for patient and family responses.

MINIMUM QUALIFICATIONS

• Bachelor's degree in nursing, healthcare, or related field.
• Current, valid RN license in practice state, or equivalent licensure (including temporary or compact licenses).
• BLS certification required.
• Specialty certification such as wound/ostomy, diabetic, or pain management preferred.
• 2-3 years of relevant clinical experience in the specialty area.
• Critical thinking, effective communication, organizational, leadership, and mentoring skills.
• Arizona Fingerprint Clearance Card required and must be maintained.

PREFERRED QUALIFICATIONS

• Master's degree in nursing preferred.
• Certifications such as CWCN, CWON, or CWOCN preferred.
• Additional education or experience related to the role.
