We are seeking a Wound Care Coordinator to join our team!

Hospital Details

Gateway Rehab, located in Florence, KY, is a 40-bed inpatient rehab providing rehabilitative services to people recovering from disabilities caused by injuries or illnesses. We help our patients regain their physical or cognitive abilities so they can return home at their highest level of independence. As a smaller, specialized hospital, we offer an opportunity to work in a fast-paced and friendly environment where you really get to know your patients and their families.

Responsibilities

Develop, implement, and evaluate care plans for patients with wound care needs. Provide consultation and education on wound, ostomy, and continence issues for patients, families, and staff. Deliver direct care and supervise wound, ostomy, and incontinence care. Demonstrate clinical competence, manage patient care, improve quality, build professional relationships, and develop clinical expertise.

Required Skills

• Active Registered Nurse license in the state of employment.
• At least 1 year of wound care experience.
• Current BLS certification from a Vibra-approved vendor.

Additional Qualifications/Skills

• Wound Care certification preferred within 6 months of hire; 2 years of experience considered in lieu.
• Wound, Ostomy, Continence (CWOCN) certification preferred; 2 years of wound care experience preferred.
• 2 years of experience in an acute hospital setting preferred.
• Knowledge of regulatory standards and compliance.
• Strong organizational and analytical skills.
• Ability to make independent decisions.
• Proficiency with computer applications.
• Drug and alcohol-free workplace.
