Responsibilities
Develop and implement team-based digital care model protocols.
Monitor program performance measures and grant partner communications.
Support evaluative grant processes with multidisciplinary teams.
Deliver educational programs on HTN control and remote monitoring.
Inform grant partners about challenges and opportunities to grow TAG model.
Collaborate with SSCHC analyst to provide performance metrics.
Support evaluative grant processes with SSCHC grant department.
Foster a strong team atmosphere and continuous improvement in TAG processes.
Build sustainability into TAG processes and ensure coordination of care.
Engage with patients for remote monitoring and outreach as appropriate.
Develop and deliver educational programs on hypertension control and remote monitoring.
Participate in grant-related meetings and organizational projects.
May act as RN team leader in ambulatory care if needed.
Participate in operations and other committee meetings as required.
Work with leadership to scale the remote monitoring program.

JOB RESPONSIBILITIES:
Facilitate the development, implementation, and ongoing adherence of team-based, asynchronous, digital care model (TAG) protocols and therapies to assure that all individuals with intermediate to high ASCVD risk receive comprehensive and timely treatment and interventions that are consistent with best practice recommendations.
Develop sustainable protocols that meet grant requirements.
Monitor program performance measures including but not limited to:
Patient volumes from initial entry to long-term chronic care initiatives
Coordination with other partners to ensure wrap-around support to enrollees
Inform grant partners of challenges and opportunities to develop a successful TAG model
Work with SSCHC analyst to provide performance metrics
Support the evaluative grant process in partnership with SSCHC grant department.
Work with the multi-disciplinary team to build a strong team atmosphere, to foster continuous improvement in current and future TAG processes.
Build sustainability into TAG processes and ensure coordination of care.
Work directly with patients as needed for remote monitoring purposes and outreach with instructions when appropriate.
Develop and deliver educational programs to help patients and their families learn more about HTN control and remote monitoring.
Act as a liaison for and participate in grant-related meetings and other organizational projects as needed.
May act as an RN team leader, supervising patient care in an ambulatory care setting.
Participate as an active member of various meetings and committees including operations/managers meetings and others as needed.
Work directly with VP of Operations, DON and other grant project leaders to grow an existing remote monitoring program into a sustainably higher level of performance and volume.
QUALIFICATIONS:
Active Nursing license in Wisconsin
Current BLS.
Minimum of 2 years clinical experience with program management experience.
Exhibits excellent interpersonal communication skills with compassion, empathy and sensitivity to participant needs.
Desire and willingness to collaborate as a member of a high-functioning transdisciplinary team.
Can work remotely as approved by SSCHC HIPAA team with in-person requirements as needed.
