Job summary

Inspired Home Health is seeking a skilled Home Health Nurse to provide in-home care across the Phoenix area. You will collaborate with physicians and the care team to implement and adjust plans of care, monitor patient status, and educate patients and caregivers on medications, safety, and symptom management.

Must hold an active Arizona RN, with home health experience, strong assessment and documentation skills, and comfort with a fully remote workflow for charting.

Qualifications

• Active Arizona RN in good standing.
• Home health or comparable clinical experience.
• Strong assessment, care-planning, and patient education skills.
• Reliable documentation and time-management in a fully remote workflow.
• Ability to support Weekends and/or Full-Time needs as scheduled.
• Must have HCHB EMR experience.

Responsibilities

Position Overview

In this Home Health Nursing role, you will provide skilled in-home nursing care to patients across Phoenix, Arizona, supporting safe recovery, comfort, and independence. You will collaborate with physicians and the interdisciplinary team to implement and update individualized plans of care, monitor patient status, and elevate changes in condition in a timely, clinically appropriate manner. Accurate, compliant clinical documentation is essential, using a fully remote workflow to ensure continuity across settings. You will also deliver clear patient and caregiver education on medications, symptom management, and home safety to reinforce adherence and reduce risk.

A Typical Shift

Your day starts by reviewing your schedule and patient notes, then organizing your route and priorities for Phoenix, Arizona. You connect with patients and caregivers, staying attentive to comfort, questions, and changing needs, and you keep communication steady with the broader care team. Between visits, you update records in a fully remote system, confirm upcoming appointments, and ensure orders and follow-ups are aligned. When Weekends coverage is needed, you help maintain dependable continuity so patients feel supported without gaps.
