StoryPoint Group in Blaine, MN is seeking a Wellness Director to oversee clinical resident care services, deliver a 1440 experience, optimize performance, and lead staff connections across the community.

The role requires a registered nurse licensed in Minnesota, CDP required within 90 days, and leadership plus revenue-management experience. You will manage care plans, coordinate Alzheimer’s programs, and ensure regulatory compliance while partnering with leaders to drive resident satisfaction.

Qualifications

• RN registered in Minnesota with CDP due within 90 days.
• Leadership experience preferred.
• Proven ability to manage revenue and financials.
• Proven high performer; able to solve complex issues.

Responsibilities

• Lead, manage, and be accountable for your team and their performance.
• Oversee revenue for Wellness, Evaluations & Care Conferences and Service Plans.
• Coordinate Alzheimer’s/Dementia program.
• Ensure compliance with 1440 care standards and state regulations.
• Communicate vision and align goals with staff and residents.
• Foster partnerships with community leaders and external partners.
• Market wellness with ED and Sales teams; evaluate admissions and care plans.
• Perform ongoing resident evaluations and update measurable goals.
• Obtain written orders for care plan changes; coordinate pharmacy care.
• Review incident reports and address personnel issues.
• Communicate emergency policies and drive continual improvement.
• Maintain flexible schedule including on-call, weekends, and holidays.
