The RN Wellness Coach is a full-time, on-site role based in Evansville, IN, focused on supporting patients in achieving and maintaining optimal health and wellness. In this role, the nurse will provide individualized coaching, education, and care coordination to help patients manage chronic conditions, adopt healthy lifestyle behaviors, and navigate available medical and community resources. Daily responsibilities may include conducting wellness assessments, developing personalized care plans, monitoring progress, and collaborating with physicians, interdisciplinary teams, and community partners to ensure continuity of care. The RN Wellness Coach will document patient interactions in accordance with regulatory and organizational standards, track outcomes, and participate in quality improvement initiatives. The role also involves delivering group education sessions, promoting preventive care, and encouraging patient engagement through clear communication and motivational support.
