Job summary

The Hospice RN Weekender at GBMC HealthCare provides nursing and supportive care to terminally ill patients and families, with weekend coverage (Saturday and Sunday 8am-8pm).

Requirements include ADN/BSN or Diploma and Maryland RN license; 1–2 years in home health or hospice preferred. Strong communication, time management, and EMR skills are essential to coordinate care with the interdisciplinary team.

Qualifications

  • Current Maryland RN license.
  • 1–2 years nursing; home health or hospice experience preferred.
  • Diploma, ADN and/or BSN from an accredited program.
  • Knowledgeable of infection control and safe nursing practices.

Responsibilities

  • Completes initial assessment of patient and family to ascertain home care needs.
  • Prepares and implements the care plan for patient/family unit in cooperation with the Interdisciplinary Group.
  • Provides direct nursing care to patients in order to maintain highest level of care.
  • Coordinates with Clinical Team Leader and community resources; updates physician monthly and at care conferences.
  • Teaches family members or hired caregivers patient care skills and medication management.
  • Arranges equipment for the home setting.
  • Attends to patient and family at time of death.

Job description

The Hospice RN Weekender reports to the Team Manager. He/she is responsible for identifying patient/family care needs, and for providing nursing and supportive care for the terminally ill patient and their families.

Education
  • Diploma, ADN and/or BSN from an accredited institution
Licensures/Certifications
  • Current Maryland State license as a Registered Nurse
Experience

One to two years of nursing. Home Health or Hospice experience preferred.

Skills
  • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection
  • Strong time management skills
  • Excellent interpersonal and communication skills
  • Ability to be patient and family focused
  • Skill in the use of EMR (Electronic Medical Record) and/or ability to learn
  • Skill in the use of Blackberry and portable computer
Principal Duties And Responsibilities
  • Completes initial assessment of patient and family to ascertain home care needs - including physical, emotional, spiritual and mental status.
  • Prepares and implements the care plan for patient/family unit in cooperation with the Interdisciplinary Group.
  • Provides direct nursing care to patients in order to maintain highest level of care for the patient in conjunction with the primary physician.
  • Cooperates with Clinical Team Leader in utilization of other team members, outside agencies, and community resources. Updates primary physician when necessary and regularly on a monthly basis. Updates interdisciplinary team at patient care conference.
  • Teaches family members or hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician, as well as addressing emotional needs.
  • Ensures that arrangements for equipment necessary in the home setting are made available.
  • Attends to patient and family at time of patient’s death.

Pay Range

$66,942.34 - $102,872.41
