Job summary

Four Seasons, Buncombe County, NC, seeks an RN to provide hospice nursing coverage to patients and families, including admissions and after-hours calls. You will work with a collaborative team to implement care plans and ensure high-quality hospice services.

The role requires active NC RN licensure, experience in hospice or clinical care, and flexibility to work nights and weekends. Comprehensive benefits and a supportive work environment are offered.

Responsibilities

• Provide nursing coverage to patients/families/community partners via phone and in-person visits.
• Assist with admission to hospice services as needed.
• Serve as first responder for after-hours symptom-based calls.
• Provide guidance to patients and families and coordinate with case manager.
• Participate in implementation and coordination of patient’s hospice care.
• Assist with emergency admissions.
• Assess physical and mental health and document nursing assessments.
• Plan, initiate, deliver, and evaluate nursing care.
• Teach, supervise, and coordinate nursing staff.
• Collaborate with other health care providers on patient plans.
• Implement prescribed regimens and medications.
• Provide health teaching and counseling.
• Document care plans and patient responses.

FT/PT/PRN: Full time with benefits

Shift: 36hour/week nights and weekends, Friday, Saturday and Sunday 8a-8p

Location: Buncombe County and surrounding areas

Position Summary:

Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

• Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
• Serve as first responder for symptom-based calls from current patients after normal office hours
• Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
• Participate in implementation and coordination of patient’s hospice care
• Assist with emergency admissions
• Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
• Record and report the results of the nursing assessment.
• Plan, initiate, deliver, and evaluate appropriate nursing acts.
• Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
• Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
• Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
• Provide teaching and counseling about the patient's health.
• Report and record the plan for care, nursing care given, and the patient's response to that care.
• Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
• Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

Knowledge, Skills & Abilities:

• Must be willing and able to care for patients with infectious disease processes unless approved accommodations
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
• Knowledge of HIPAA privacy rules and regulations

Minimum Qualifications:

Experience:

• Two years of clinical experience preferred
