Job summary

Autumn Leaves Nursing & Rehab is seeking an experienced RN Weekend Supervisor to oversee daily nursing activities on weekends and manage wound care. This part-time role requires an active Texas RN license, EMR experience, and the ability to supervise direct care staff while maintaining high-quality patient care on Saturdays and Sundays.

The Weekend Supervisor coordinates with physicians, reviews reports, and ensures safety, infection control, and regulatory compliance.

Qualifications

• Unencumbered Registered Nurse (RN) license in Texas.
• Weekend Supervisor experience in SNF preferred.
• EMR experience required; PCC preferred.
• Ability to provide age-appropriate care.
• Be a supportive team member and promote teamwork.
• Ability to make independent decisions when needed.
• Able to interact tactfully with residents, families and staff.

Responsibilities

• Oversee day-to-day nursing activities during assigned weekend shifts.
• Obtain shift report and relay to the oncoming nurse; document follow-up actions.
• Conduct resident rounds daily and escalate issues to supervisor.
• Identify and address safety hazards immediately.
• Assess emergencies and initiate physician-directed care in absence of a physician.
• Supervise direct care associates providing resident care.
• Monitor narcotic procedures and shift documentation.
• Notify physician of changes in condition or abnormal results as needed.
• Document daily notes for unstable residents and Medicare patients.
• Ensure resident and staff safety and observe infection control procedures.
• Perform wound treatments as assigned and any other duties as needed.

We are looking for an experienced individual to become part of Autumn Leaves' clinical team as a Weekend Supervisor (RN)! The Weekend Supervisor oversees the day-to-day nursing activities of the Community during their assigned shifts and completes wound treatments.

Position and Schedule

• RN Weekend Supervisor
• Part-Time
• Schedule: Saturday - Sunday

Responsibilities

• Follow established standards of nursing practices and implement community programs, policies, and procedures.
• Obtain report from nurse being relieved and provide report to nurse coming on duty; record sufficient information to implement appropriate follow-up action, as necessary.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Identify safety hazards and initiate corrective action immediately.
• Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician.
• Provide supervision to direct care associates providing direct resident care.
• Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereof.
• Notify physician, when necessary, of changes in conditions, weight, medication errors, medication not available, abnormal lab/X-Ray results, and any other unusual observations
• Document daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residents.
• Be responsible for the safety of residents and associates under his/her supervision.
• Observe infection control procedures.
• Perform all the duties of a certified nursing assistant when required.
• Must perform all wound treatments assigned.
• Other duties as assigned.

Requirements

• Unencumbered Registered Nurse (RN) License in Texas
• Previous Weekend Supervisor experience in SNF highly preferred
• Electronic Medical Record Experience required, PCC preferred
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

Qualifications

• 1 year experience as an RN
• Compact license or Texas Nursing License in good standing
• Proof of current CPR certification
• Ability to work flexible schedule
• Excellent interpersonal skills
• Ability to work a flexible schedule
