Job summary

Affinity Hospice is seeking a dedicated Registered Nurse for weekend on-call Full Time in Canton, GA. The RN will lead clinical care in home settings, coordinate interdisciplinary teams, and ensure quality patient outcomes while supporting families.

The role requires an RN license, CPR, and 2+ years of RN experience in hospice or home health. A valid driver’s license and reliable vehicle are required.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State nursing license.
• Current CPR certification.
• Minimum of two years RN experience in clinical care, home health or hospice setting preferred.
• Must be a licensed driver with a reliable automobile.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team and family.
• Implement, develop and document the patient plan of care.
• Provide care with infection control measures and OSHA standards.
• Ensure continuous quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call duties as assigned by management.
