Job summary

CommonSpirit Health at Home in Fort Wayne, IN is seeking a Registered Nurse to provide compassionate, patient-centered home health care in patients' homes. You will deliver one-on-one nursing care, assess needs, and adjust plans with input from physicians, therapists, and support staff.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
