Job summary

Home Health Care, Inc. in Minnesota is seeking an RN Visit Nurse to deliver skilled home health visits in patients' homes across St. Paul and surrounding communities. This role emphasizes 1:1 patient care, comprehensive assessments, and personalized care planning.

You will collaborate with clinicians, educate families, and document visits using Homecare Homebase, with strong support from clinical leadership and a culture that values quality patient care over rushing visits.

Qualifications

• Must hold an active Minnesota RN license
• Experience in home health or similar settings preferred
• Strong clinical decision-making and communication skills will be relied upon to educate patients and families

Responsibilities

• Provide skilled home health nursing visits in patients' homes
• Complete comprehensive assessments and ongoing evaluations
• Perform wound care, medication management, injections, and disease monitoring
• Develop and update individualized care plans
• Educate patients and caregivers to improve health outcomes
• Coordinate care with physicians and interdisciplinary team members
• Document visits accurately in Homecare Homebase
• Identify changes in patient condition and intervene appropriately

Job Title: RN Visit Nurse – Home Health | St. Paul Territory | $4,000 Sign-On Bonus

Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

What You'll Be Doing

Who We're Looking For

• Active Minnesota Registered Nurse (RN) license


• Experience in home health, hospital, rehabilitation, skilled nursing, or acute care


• Strong assessment, critical thinking, and clinical decision-making skills


• Compassionate communicator who enjoys educating patients and families


• Ability to work independently while managing a daily schedule


• Reliable transportation and willingness to travel throughout the St. Paul service area
