Job summary

Home Health Care, Inc. is seeking an RN Visit Nurse for a St. Paul territory to provide compassionate, skilled care in patients' homes. The role offers a strong 1:1 patient relationship, with autonomy supported by experienced leaders and a Medicare-certified agency with over 30 years in Minnesota.

Primary duties include home health visits, assessments, wound care, and education, with collaboration across therapists, physicians, and case managers to improve outcomes.

Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Experience in home health, hospital, rehabilitation, skilled nursing, or acute care.
• Strong assessment, critical thinking, and clinical decision-making skills.
• Compassionate communicator who educates patients and families.
• Ability to travel throughout the St. Paul service area.

Responsibilities

• Provide skilled home health nursing visits in patients' homes.
• Complete comprehensive assessments and ongoing evaluations.
• Perform wound care, medication management, injections, and disease monitoring.
• Develop and update individualized care plans.
• Educate patients and caregivers to improve health outcomes.
• Coordinate care with physicians and interdisciplinary team members.
• Document visits accurately in Homecare Homebase.
• Identify changes in patient condition and intervene appropriately.

• Ability to work independently while managing a daily schedule
• Reliable transportation and willingness to travel throughout the St. Paul service area
