Job summary

Good Samaritan University Hospital in West Islip, NY, part of Catholic Health, seeks a Registered Nurse for Admission/Discharge to join a collaborative nursing team on-site. You will facilitate timely admissions and discharges, complete assessments, and provide education to patients and families to support safe transitions.

The ideal candidate holds a BSN (ASN considered if enrolled in BSN), NY RN license, and BLS.

Qualifications

• BSN required for licensure and practice.
• ASN candidates active in BSN progression will be considered.
• Current NY RN license required (Professional Nurse).
• BLS certification from American Heart Association required or equivalent.

Responsibilities

• Facilitate admissions and discharges in a timely, patient-centered manner.
• Complete required admissions/discharge assessments and documentation.
• Provide individualized education and support to patients and families.
• Assess, plan, implement, and evaluate nursing care using clinical knowledge and judgment.
• Collaborate with the interdisciplinary team to coordinate care and transitions.
• Delegate appropriate patient care activities while maintaining accountability for nursing care.
• Address age, language, culture, health literacy, and developmental/physical needs in patient care.
• Maintain patient safety, infection prevention, and standards of care.
• Maintain professional competency and participate in ongoing education.
• Participate in unit-level performance improvement initiatives.

• Facilitate seamless, timely and patient-centered admissions and discharges.
• Complete required assessments and admission/discharge documentation.
• Provide individualized education and support to patients and families.
• Assess, plan, implement and evaluate nursing care using sound clinical knowledge and judgment.
• Collaborate with the interdisciplinary team to coordinate care and support safe transitions.
• Delegate appropriate patient care activities while maintaining responsibility and accountability for nursing care.
• Respond appropriately to individual patient needs, including age, language, culture, health literacy and developmental or physical needs.
• Maintain patient safety, infection prevention practices and established standards of care.
• Maintain professional competency and participate in continuing education relevant to the clinical area.
• Participate in unit-based performance improvement initiatives and support departmental goals.

Qualifications

• Bachelor of Science in Nursing (BSN) required; candidates with an Associate Degree in Nursing (ASN) who are actively enrolled in a BSN program will also be considered.
• Current New York State Registered Professional Nurse license required.
• Current BLS certification from the American Heart Association required.
• Previous patient care experience in a hospital setting preferred.
• Strong communication, clinical assessment, prioritization and patient education skills.
• Proficiency with electronic medical records (EMR, Epic preferred) and computer systems.

Salary Range

USD $56.39 - USD $73.66 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
