The Registered Nurse- Patient Aligned Care Team position is located at the Washington Avenue VA Clinic - VA St. Louis Health Care System - St. Louis - MO.

Qualifications

• Graduate of a nursing program approved by accreditation bodies (ACEN/CCNE).
• Eligibility for State nursing licensure is required or in progress.
• Foreign-educated nurses may require degree equivalency validation.
• Current, full, active, unrestricted nursing registration in the U.S. or DC.
• Knowledge of VA qualification standards and clinical leadership desirable.

Responsibilities

• Provide fundamental, knowledge-based nursing care to assigned clients.
• Coordinate care with education, self-management, and transitions.
• Assess, plan, implement, and evaluate patient care per standards.
• Lead care coordination and health education for veterans in primary care.
• Delegate tasks to team members based on patient needs and safety.
