Job summary

Jefferson Health is seeking a professional nurse to deliver patient- and family-centered care using the nursing process. The RN will develop care plans, address physical, psychological, emotional, and social needs, and collaborate with patients and care partners to meet priorities consistent with cultural beliefs.

Minimum qualifications include a specialized nursing diploma and PA RN licensure; BSN is preferred and acute care experience is valued.

Responsibilities

  • Interacts with co-workers, patients, their family/designated care partner(s), and other staff in a manner consistent with Jefferson values.
  • Integrates Evidence-Based Practice, clinical expertise, patient preferences, and cultural beliefs into care delivery.
  • Partners with patients and families in identifying learning needs and priorities and provides education.
  • Minimizes risk by following nursing practices, protocols, and performance standards.
  • Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decisions.
  • Uses data to monitor care outcomes and apply improvements for quality and safety.
  • Works within nursing and inter-professional teams, fostering open communication and shared decision-making.
  • Recognizes patients and families in providing safe, compassionate, coordinated care based on preferences and culture.
  • Creates an environment of belonging with problem-solving and conflict-resolution skills.
  • Demonstrates willingness to learn and act as a preceptor when appropriate.

Job Overview

The nurse develops nursing plans of care with the patient and their designated care partner(s) to effectively meet individualized needs and priorities consistent with their cultural beliefs and values.

Minimum Qualifications
  • Specialized Diploma or accredited school of nursing graduate required.
  • Patient‑and Family‑Centered Practice knowledge and ability to convey respect and collaboration.
  • Cultural awareness and ability to connect effectively with diverse backgrounds.
  • Effective communication skills, understanding of concepts, tools, and techniques for accurate transmission and interpretation.
  • Patient assessment and care knowledge, including implementation of treatment plans.
  • Patient and Designated Care Partner education knowledge and ability to elicit learning priorities.
  • Medication administration and prescription knowledge of various medications.
  • Discharge planning knowledge and ability to partner with patients and families in developing discharge plans.
  • Assessment and diagnosis knowledge of tools and techniques for nursing diagnosis or clinical judgment.
  • Health information documentation knowledge and ability to record medical histories, vital signs, medications, diagnoses, treatment plans, procedures, tests, and results.
  • Licensed Registered Nurse or PA licensed in the State of Pennsylvania.
Preferred Qualifications
  • Bachelor’s Degree (BSN) or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred.
  • One year previous experience as an RN in an acute care setting strongly preferred.
Additional Qualifications
  • Physical demands: lift and carry 40‑50 lbs, push/pull patients on bed or stretcher (requires 29 lbs push force), lateral transfers up to 50 lbs of patient weight, frequent to continuous standing/walking, walk 8‑10 miles per shift. May require kneeling, bending, squatting, and performing CPR.
