Job summary

Garden Gate Health Care Facility is seeking an experienced RN Unit Coordinator to oversee the clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, staff supervision, and interdisciplinary coordination.

Located in Cheektowaga, NY, this full-time position offers a salaried schedule with a 7 am-3:30 pm shift, no weekends, on-call rotation, and a sign-on bonus up to $7,500 to join The McGuire Group’s trusted long-term care team.

Qualifications

  • Associate degree in nursing required; NYS RN license required.
  • 1 year minimum RN experience in long-term care.
  • 3–5 years LT care with supervisory experience preferred.
  • Strong knowledge of resident assessments and care planning.
  • Leadership experience with coaching and mentoring staff.

Responsibilities

  • Oversee the clinical care and daily operations of an assigned nursing unit.
  • Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff.
  • Conduct resident assessments and ensure care plans are implemented and followed.
  • Monitor resident conditions and communicate changes with providers and families.
  • Review orders, labs, incident reports, and quality indicators.
  • Participate in care plan meetings, family meetings, and interdisciplinary rounds.
  • Coordinate admissions, discharges, transfers, and resident services.
  • Ensure accurate documentation, medication management, and regulatory compliance.
  • Support staff education, orientation, coaching, and performance management.

Equivalent Hourly: $34- $38
Sign-on Bonus: Up to $7,500

RESPONSIBILITIES:

  • Oversee the clinical care and daily operations of an assigned nursing unit
  • Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff
  • Conduct resident assessments and ensure care plans are implemented, updated, and followed appropriately
  • Monitor resident conditions and communicate changes with providers, families, and interdisciplinary team members
  • Review provider orders, laboratory results, incident reports, and quality indicators
  • Participate in care plan meetings, family meetings, QAPI activities, and interdisciplinary rounds
  • Coordinate admissions, discharges, transfers, outside appointments, and resident services
  • Ensure accurate documentation, medication management, and regulatory compliance
  • Support staff education, orientation, coaching, and performance management activities
  • Monitor infection control, immunization tracking, resident safety, and quality improvement initiatives
  • Address resident, family, and staff concerns while promoting a positive care environment
  • Perform additional duties as assigned

REQUIREMENTS:

  • Associate Degree in Nursing required; Bachelor's Degree preferred
  • Valid NYS Registered Nurse (RN) License required
  • Minimum 1 year of RN experience in a long-term care setting required
  • 3-5 years of long-term care experience with supervisory experience preferred
  • Strong knowledge of resident assessments, care planning, and long-term care regulations
  • Leadership experience with the ability to supervise, coach, and mentor staff
  • Excellent communication, organization, and problem-solving skills
  • Ability to coordinate care across multiple disciplines and manage competing priorities
  • Experience with quality improvement, infection prevention, and regulatory compliance preferred
  • Commitment to providing compassionate, resident-centered care
