RN Uniform System Assessor Field Travel in NY (Bronx, Buffalo, Manhattan, Syracuse, Albany, Suffolk)

Join Molina Healthcare as an RN Uniform System Assessor supporting our community health initiatives. This remote position requires field travel within designated areas based on your residence.

Job Summary

Responsible for completing Community Health Assessments (CHA), formerly UAS (Uniform Assessment System), including Initial Assessments and Reassessments, in compliance with New York State requirements. Duties include processing enrollment paperwork, utilizing the CHA Tasking Tool, and revising documentation based on quality reviews. Support our Senior Whole Health business, a Managed Long-Term Care (MLTC) and Medicaid Advantage plan serving chronically ill or disabled members eligible for Medicaid and Medicare.

Key Responsibilities
  • Complete health assessments and develop care plans.
  • Conduct home visits to gather data, complete enrollment paperwork, and ensure data accuracy.
  • Document contacts and findings within designated tools and forms.
  • Attend training sessions on assessment documentation and enrollment procedures.
  • Participate in quality improvement initiatives and support performance improvement activities.
  • Apply principles of Care Management and Person-Centered Planning.
  • Travel to multiple boroughs via car or public transportation as needed.
Qualifications
  • Associates degree in Nursing.
  • Minimum two years of clinical experience, including managed care, disease or case management, and home visits.
  • Current RN licensure in NY.
  • Residency in NY or neighboring states (NJ, CT, parts of PA) with NY state ID.
Preferred Qualifications
  • Bachelor’s degree in Nursing.
  • Certifications such as CCM or CCP.
  • Experience in long-term care, MLTC, utilization review, or risk management.
  • Bilingual skills in Spanish, Chinese, or Bengali.
Work Details
  • Hours: Monday-Friday, 8:00am-5:00pm EST.
  • Travel: Approximately 50% driving.
  • Compensation: $24 - $56.17 hourly, varies by location and experience.
