This phone triage position is per diem/on call/as needed.


The Triage RN provides expert geriatric professional, independent nursing care for Providence ElderPlace patients that is as evidence-based as possible. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. The RN applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. All Providence professional nursing practice requires a broad range of general geriatric nursing knowledge and expertise. In addition, development and validation of knowledge, expert geriatric nursing knowledge and prioritization of skills and critical thinking essential to the assigned department, patient population and/or practice area are crucial.

Required Qualifications:



  • Education sufficient for licensure.

  • Oregon Registered Nurse License upon hire.

  • National Provider BLS - American Heart Association within 30 days of hire.

  • In compliance with company policy and state regulations, a Department of Human Services (DHS) background check may be required based upon the location of the position. Fingerprinting may also be required upon hire.


Preferred Qualifications:



  • Bachelor's Degree - Nursing.

  • 3 years - RN experience in PACE, Home Health, Skilled Nursing Facility, Hospice, RN Call/Advice Center or ER.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
