Job summary

Job Summary

Transplant coordinator coordinates solid organ (liver, kidney, pancreas, and multi‑organ) transplantation for all pediatric and adult recipients. This clinical, educator, investigator, consultant, and leader role is a primary member of a multidisciplinary team responsible for organizing care of transplant patients and providing follow‑up throughout the continuum of care.

Education & Experience

Licensed as RN for at least one (1) year.

Required Certifications and Licenses

Valid RN license. Basic Life Support (BLS) required.

Preferred Qualifications

• Nursing experience in an intensive care environment, related nursing experience, or transplant experience.
• BSN preferred.
• Certified Clinical Transplant Coordinator (CCTC) and ACLS preferred.

Responsibilities

• Provide daily clinical assessment of transplant patients throughout all phases of care.
• Coordinate evaluation process for pre‑transplant (referral) patients to ensure effective use of hospital and clinical resources.
• Assess post‑transplant organ function and general health status and determine need for additional assessment or intervention.
• Facilitate scheduling of studies or procedures for diagnostic purposes, managing fiscal responsibility for patient and hospital costs.
• Participate in development and implementation of protocol and standards of care for transplant patients with the multidisciplinary team.
• Assist nurses and other staff to access, plan, and implement and evaluate care of transplant patients.
• Monitor process and outcomes of routine care to determine quality outcomes and performance improvement processes.
• Provide on‑call coverage, including coordination of activities prior to transplant and as medical triage for outpatients in collaboration with the physician/provider on‑call.
• Coordinate transplantation processes when a donor is available (24h/7d) including communication between physicians, organ procurement organizations, patient/family, and medical teams such as ED, ICU, blood bank, and histocompatibility laboratory.
• Review patient's medical care including lab and procedural results, maintain medication reconciliation, assess patient issues, and triage care with transplant providers.
• Complete necessary documentation according to regulatory requirements per CMS and UNOS.
• Communicate timely with multidisciplinary team and patients/family regarding all care issues.
• Collaborate effectively with all multidisciplinary team members and care teams involved with the transplant patient.
• Maintain patient transplant record with current data to facilitate communication.
• Plan nursing care and medical procedures with fiscal responsibility by working collaboratively with transplant administrators, financial coordinators, health system utilization review, and third‑party case managers.
• Educate potential recipient and donor candidates, their families, and caregivers throughout the continuum of care.
• Develop patient education materials or identify external resources for patient education in all phases of transplantation care.
• Educate nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for transplant patients.
• Provide education and consultation about transplantation and organ donation for the community at large.
• Serve as a resource person/preceptor for graduate and undergraduate nursing programs.
• Participate or lead team conferences, rounds, or meetings related to care of the transplant patient or program issues.
• Interpret research and published data to nursing staff regarding current issues in transplantation and incorporate research into evidence‑based practice.
• Identify areas for study and participate in and/or design and conduct research.
• Serve as a consultant to nurses and other health care professionals regarding care of the organ transplant patient.
• Advising referring physicians/nurses in collaboration with surgical and medical teams.
• Provide community consultation resource regarding transplantation and organ donation.
• Consult with unit leadership to resolve issues related to transplant patients.
• Demonstrate professional nursing through conduct, appearance, communication, respect, leadership, ethical decision‑making, critical thinking, and problem‑solving skills.
• Support and participate in strategic plan for university transplant, UMMC, nursing, and ancillary support services.
• Participate in university transplant departmental committees, hospital‑wide committees or task forces or meetings as indicated.
• Participate in organ transplant program planning.
• Enhance body of knowledge in nursing and solid organ transplantation through presentations, publications, and continuous education.

Physical and Environmental Demands

Exposure to high noise level, heat and cold, biohazardous conditions, occasional traveling, and other duties including bending, lifting up to 25pounds, pushing/pulling, reaching, frequent standing, and constant walking.
