Transitions Care is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. You will deliver skilled nursing focused on symptom management, comfort, and quality of life for patients with serious or terminal illness.

Qualifications

• Active RN license in good standing.
• Experience in hospice or palliative care preferred.
• Strong communication with patients and families.
