Job summary

Endeavor Health is seeking an RN Transitional Care Navigator to manage case and utilization across care settings. You will coordinate post-acute care, facilitate safe transitions, and partner with the healthcare team to ensure timely decisions and discharge planning.

This role emphasizes improving cost, readmission rates, and SNF utilization. The position requires an active RN license, and experience in management within skilled nursing facilities is preferred.

Qualifications

• Must hold a valid RN license.
• Certification: case management or rehabilitation nurse certification preferred.
• Experience: minimum 3 years in a management role in a skilled nursing facility preferred.
• Knowledge of Epic, Point Click Care and basic Microsoft Office programs

Responsibilities

• Support high-risk patients discharged to skilled nursing facilities and manage length of stay.
• Guide patients and families through the health system from diagnosis to follow-up care.
• Coordinate care transitions and ensure timely discharge planning.
• Analyze data to identify under/over utilization and improve cost and quality of care.
