Job summary

Centene Corporation seeks a registered nurse with California licensure to manage post-discharge care and coordinate medication reconciliation. The role focuses on evaluating needs, planning care, and collaborating with discharge planners and providers to support transitions.

Requires a master’s degree (Behavioral Health, Social Work) or an accredited nursing degree and 2–4 years of related experience. Preference for CA residents able to work PST hours.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a Nursing degree from an accredited school.
• 2–4 years of related experience.
• Active RN license in California.
• Preference for CA residents who can work Pacific Time hours.

Responsibilities

• Evaluates the needs of the member by completing post-discharge assessments.
• Evaluates medication and performs reconciliation between pre-admit and post-discharge medications.
• Develops a care/service plan and collaborates with discharge planners, providers, specialists, and teams.
• Assesses member health status, resources, and plans and provides interventions.
• Facilitates transition into active care management based on member needs.
• Provides or facilitates education and resources to promote wellness.
