RN, Transition Care Unit – High Point Medical Center, Atrium Health Wake Forest Baptist, North Carolina

We are seeking a registered nurse (full‑time, days) to join our Transitional Care Unit. As a pivotal member of our dynamic team, you will deliver exceptional patient care and positively impact patient outcomes.

Position Highlights

• Sign‑on bonus: $10,000 for qualified candidates
• Shift schedule: Full time, days (7:00 am – 7:30 pm) with weekend rotation
• Department: Transitional Care Unit
• Location: High Point Medical Center

Job Responsibilities Include

• Patient/Family Centered Care: Utilize the nursing process to deliver compassionate care, placing patients and families at the center of every decision. Conduct thorough assessments, develop nursing diagnoses, identify outcomes, plan interventions, and coordinate care to promote optimal health and well‑being.
• Patient Discharge & Referrals: Participate in multidisciplinary discharge planning and referrals to hospital and community resources as needed.
• Safe, Therapeutic, and Healing Environment: Create a secure and healing environment by safeguarding patient privacy and ensuring responsive, respectful, and professional behavior.
• Effective Communication: Demonstrate excellent communication skills to foster collaboration among the healthcare team and facilitate information sharing throughout the continuum of care.

Pay Range

$34.90 – $52.35 per hour
