Job summary

ProMedica is seeking an RN Acute Care Navigator to oversee patient care, optimize care progression, and coordinate discharge planning for assigned caseload in Toledo, OH. This role emphasizes reducing length of stay, preventing readmissions, and ensuring value-based care compliance.

The position requires an active RN license in the state, with BSN or 5 years of nursing experience. MSN or higher is preferred, with CCM/RN-BC/ACM or similar certifications valued.

Qualifications

• Must have either a BSN, or 5 years’ applicable nursing experience.
• A current, active, and unrestricted licensure as a Registered Nurse in the state of practice is required.
• MSN or higher desired/preferred.

Responsibilities

• Oversee patient care, seamless care progression, coordination, and appropriate level of care for caseload.
• Collaborate with interdisciplinary teams to conduct initial assessments and develop discharge plans.
• Provide patient education on medications, treatments, and post-discharge care.
• Monitor patient progress, review daily health records, and escalate issues to leadership to ensure smooth transitions and outcomes.
