Job summary

Beth Israel Lahey Health at Home at Boston area is seeking a per diem Registered Nurse to plan, organize and direct home health services with emphasis on community health education.

You will complete initial patient assessments, develop and revise plans of care, administer medications, supervise home health aides, and document in EPIC and OASIS to support timely, compliant care.

Qualifications

• Active Massachusetts RN license.
• Active American Heart Association BLS certification.
• Associate’s Degree or BSN (preferred) from an accredited school of nursing.
• At least 1 year of direct care nursing experience, preferably in public health or home care nursing.
• OASIS and/or EPIC experience is preferred.

Responsibilities

• Plan, organize and direct home care services.
• Complete and document an initial assessment of patient and family.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Instruct, supervise and evaluate the home health aide care provided every two weeks.
• Identify discharge planning needs as part of care plan development.
• Ensure HIPAA compliance and accurate documentation in EPIC.
