Job summary

Beth Israel Lahey Health at Home is seeking a dedicated Registered Nurse for per diem day shifts in the Watertown area. You will plan, organize, and direct in-home care, performing initial assessments and developing care plans in collaboration with patients and families.

Responsibilities include administering medications, guiding home health aides, and ensuring HIPAA-compliant documentation in EPIC while adhering to payer guidelines and home care regulations.

Qualifications

• Massachusetts RN license active.
• Active BLS certification.
• Associate’s degree or BSN from an accredited nursing program (preferred).
• At least 1 year of direct care nursing experience, ideally in home care/public health.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiate the plan of care and revise as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care provided every two weeks.
• Identify discharge planning needs as part of care plan development.
• Maintain HIPAA confidentiality and agency policies; ensure timely documentation in EPIC.
