Job summary

Trilogy Health Services, LLC is seeking a Registered Nurse (RN) to lead a care team and deliver high-quality nursing care at our Woodmont Health Campus in Boonville, IN. This role emphasizes medication management, documentation, and patient-centered care on a daily basis.

The RN will oversee care planning, perform nursing assessments, and escalate changes in residents' conditions to the charge nurse and physician as needed. A current Indiana RN license and CPR certification are required.

Qualifications

• Must hold a current, valid RN license.
• CPR certification required.
• Long-term care experience is preferred.

Responsibilities

• Lead a team of direct care providers to ensure proper medications, treatments, and documentation.
• Apply the nursing process to deliver patient care and ensure continuity of care from admission to discharge.
• Visit residents daily to assess physical and emotional status and notify nursing leadership of changes.
• Identify resident problems and emergencies and initiate lifesaving measures when needed.
• Perform administrative duties such as forms, reports, evaluations, and charting.
