SUMMARY/PURPOSE OF JOB
The Supplemental Wound Care Nurse provides consultation to all medical, nursing and therapy staff for complex wound and ostomy patients according to hospital referral criteria and National Alliance of Wound Care standards.
The incumbent also instructs patients and families in procedures and health care practices and makes referrals for continuing care after discharge. As an RN, the Wound Care Nurse provides care to patients from fifteen years of age and older based on age-specific need.
Understands and exhibits the principles of service excellence, leads by example, and mentors others in the performance of their assigned duties. Additionally, understands that every employee
is responsible for exhibiting exceptional customer service by answering call lights on the nursing unit and if not able to assist personally, will inform the appropriate employee regarding the
patient's need
Understands the role of employee's individual position regarding HIPAA requirements and level of access to Protected Health Information as defined in HIPAA hospital-wide policies.
JOB DUTIES/ESSENTIAL FUNCTIONS
I. PRIMARY JOB DUTIES/ESSENTIAL FUNCTIONS
Provides consultative, technical, clinical and educational support to the physicians and staff of RHI and the patient regarding treatment and management of wounds and
ostomies, providing input for policies and procedures and assisting with the development of wound care competencies for licensed staff.
Documents initial and ongoing objective and subjective data in the EMR, including both photographs and wound measurements in order to provide evidence of the healing
process or lack thereof in order to update the treatment plan. Assists interdisciplinary team to develop both short- and long-term goals to support wound healing process.
Assists in the review and piloting of wound care products in order to promote standardization throughout the house, working with both the vendors and Materials Management to improve cost effectiveness and improve outcomes.
Demonstrates recognition of the individual role in RHI safety, risk management, policy and QI/PI programs. Demonstrates recognition of legal implications by appropriate
documentation of care. Utilizes materials/resources in a cost-effective manner.
Demonstrates assertive role in personal/professional growth; actively participates in and documents the orientation process, formulates goals, attends mandatory CPR and other
required certifications and competencies.
Assumes accountability for own actions congruent with standards, etc., incorporating new information into clinical practice with assistance. Collects data as directed for
research projects.
Participates in performance improvement/ program evaluation activities and observes measures to ensure Hospital, Joint Commission, and other quality standards and
regulations are met.
Uses appropriate technique in performing duties to avoid or minimize accidents and injury related to unsafe practice. Proactively seek information and resources, including the
Environment of Care Manual, to aid in safe performance of duties to contribute to the RHI culture of safety.
Employee is focused on providing customer satisfaction (including, but not limited to, patients, co- workers, visitors, vendors and physicians) as a primary goal/outcome of all
interactions; places customer first in the performance of job duties and responds to needs in a timely manner as evidenced by the absence of legitimate customer complaints.
Performs offsite consultative services, as requested, assessing a patient for wound treatment needs and tolerance to therapy, including patient direct observation and
assessment under the supervision of the referring facility treatment team.
The incumbent completes formal ongoing competency evaluation successfully, maintain and demonstrates an acceptable level of clinical competency in the care of the mid
adolescent. Adult and geriatric physical medicine rehabilitation population served from age 16 upward.
Other duties as assigned.

II. EFFECT OF ERRORS
IN PERFORMING PRIMARY JOB DUTIES/ESSENTIAL JOB FUNCTIONS:
The incumbent could cause death or other negative outcomes directly by administering wrong medication or treatment to patient, by failure to give appropriate attention to patient complaints or
request, or by failure to use equipment correctly. Failure to observe and report changes in patient suspected of infection, etc. could cause patient discomfort or death. Failure to carry through or
ensure proper follow-through of patient discharge plans, continuity of care plans, etc., could result in legal and financial complications for the Hospital. A careless, tactless, and/or unsympathetic
manner with patients, relatives, visitors, medical staff, etc., could injure Hospital's image. Poor judgment in handling employee problems could create feelings of ill will and dissatisfaction among
employees.
QUALIFICATIONS
NECESSARY TO PERFORM PRIMARY DUTIES/ESSENTIALJOB FUNCTIONS:
KNOWLEDGE, SKILLS, ABILITIES This position requires knowledge of the qualifications, duties and responsibilities of a professional nurse and responsibilities of non-professional
personnel as acquired during clinical nursing experience at an approved School of Nursing. Requires an ability to assess the patients' needs (acute care, rehab, and age dependent), and to
plan, direct and coordinate nursing patient care, and an ability to instruct other personnel and the patient and family concerning patient care and health practices, Must maintain an awareness and
understanding of legal implications of all patient care activities, own practice and of employee relationships and activity. The incumbent demonstrates appropriate knowledge of discharge planning and is aware of contacts and/or community resources available to patients and families. Must display the ability to preserve patient dignity, privacy, and confidentiality. Must be able to
communicate and interact with patients, families, hospital personnel and preserve confidentiality. Must be able to direct and supervise others in providing patient care activities, specializing in
wound management.
EXPERIENCE
This position requires experience gained through an approved School of Nursing with an appropriate period of orientation for the performance of nursing care. Direct
patient care positions require direct patient care experience within the preceding five years. Incumbents must successfully complete nursing orientation objectives.
SPECIAL TRAINING, LICENSURE, CREDENTIALING
This position requires current State of Indiana licensure as a Registered Nurse. Must receive CPR training in accordance with American Heart Association Standards resulting in receipt of course completion card upon hire and maintain current thereafter.
EDUCATION
Graduate of accredited School of Nursing. A Bachelor of Nursing degree is encouraged.
