Job summary

Apploi's skilled nursing facility in the Bronx County, NY is hiring full and part-time RN Supervisors for all shifts to oversee day-to-day clinical activities of LPNs and CNAs, interpret patient information, and make critical decisions.

The RN Supervisor will administer medications, wound care, and other personalized interventions, participate in care planning, and mentor staff while ensuring DOH infection control standards are met.

Qualifications

• Current NYS Registered Nurse license.
• 1–2 Years RN Supervisor experience in a Nursing Home setting.
• Current BLS certificate preferred.

Responsibilities

• Oversee day-to-day clinical activities of LPNs and CNAs.
• Oversee admissions assessments and reassessments for all patients.
• Provide leadership and supervision to nursing staff, housekeeping, and security.
• Collaborate with medical support staff to meet resident needs.
• Greet admitted patients and prepare them for examinations.
• Maintain medical supplies and equipment.
• Educate staff based on patient learning needs.
• Assess physical and psycho-social conditions throughout care.
• Record patient information accurately in EMR.
• Maintain infection control standards per agency and DOH.
• Coordinate patient care with nurses, physicians and allied health professionals.
• Participate in interdisciplinary team case conferences.
• Ensure adherence to policies and procedures.
• Document changes in residents' conditions.
• Participate in care planning meetings.
• Mentor and supervise LPNs and CNAs.

Job description

Skilled Nursing facility, located in the Bronx County, New York, is actively looking to hire clinically superb full and part time RN Supervisors for all shifts. The RN Supervisors will oversee the day-to-day clinical activities of health care personnel, such as LPNs and CNAs. The RN Supervisor must be able to interpret patient information and make critical decisions about necessary actions, as well as administer medication, wound care and other personalized interventions.

• Completes admissions assessments and reassessment on all patients.
• Responsible for providing leadership and supervision to the nursing staff, housekeeping, and security.
• Works closely with medical support staff to ensure resident needs are met.
• Will greet admitted patients and prepare them for physical examination and treatment.
• Will maintain medical supplies and equipment.
• Provides education as indicated after assessing the learning needs of the patient, staff members.
• Assess the patient’s physical and psycho-social condition on an ongoing basis throughout the continuum of care.
• Utilize manual and automated information systems to improve efficiency and effect productivity. Accurately records patient information in the electronic medical records system and patient medical record.
• Demonstrate a current working knowledge of equipment, instrumentation and medications.
• Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals.
• Participate in preparation and facilitation of regulatory agency site visits to include DOH
• In accordance with agency protocol and DOH standards, maintains adequate infection control standards.
• Reviews and reports all medical, accident/incidents related to patient care and provides input into actions necessary to resolve problems.
• Participate in weekly and on an “as needed basis” case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns.
• Ensure that personnel adhere to established policies and procedures.
• Document changes in resident's conditions.
• Participate in care planning meetings.
• Mentoring and supervision of LPN's and CNA's.

Requirements:

• Current NYS Registered Nurse license.
• 1-2 Years RN Supervisor experience in a Nursing Home Setting required.
• Current BLS Certificate preferred.
