Job summary

Sapphire Nursing at Meadow Hill is seeking RN Supervisors for the 3pm to 11pm shift to oversee day-to-day clinical activities of LPNs and CNAs in a skilled nursing facility located in Orange County, NY.

The role requires interpreting patient information, making critical care decisions, administering medications and wound care, and coordinating with a multidisciplinary team to meet resident needs with a focus on regulatory compliance and quality of care.

Qualifications

• Current NYS Registered Nurse license.
• 3 Years RN experience preferred.
• Current BLS Certificate.

Responsibilities

• Completes admissions assessments and reassessment on all patients.
• Provides leadership and supervision to the nursing staff, housekeeping, and security.
• Works with medical support staff to ensure resident needs are met.
• Greet admitted patients and prepare them for physical examination and treatment.
• Maintain medical supplies and equipment.
• Provide education as indicated after assessing learning needs of patient and staff.
• Assess patient’s physical and psycho-social condition throughout the continuum of care.
• Record patient information accurately in EMR and patient medical records.
• Demonstrate knowledge of equipment, instrumentation and medications.
• Coordinate patient care activities with nurses, physicians and allied health professionals.
• Assist in regulatory agency site visits including DOH.
• Maintain infection control standards as per protocol and DOH standards.
• Review and report medical, accident/incidents related to patient care and propose actions.
• Attend weekly and ad-hoc case conferencing with interdisciplinary team.
• Ensure adherence to policies and procedures.
• Document changes in residents’ conditions.
• Participate in care planning meetings.
• Mentor and supervise LPNs and CNAs.

Sapphire Nursing at Meadow Hill, a skilled nursing facility, located in Orange County, New York is actively looking to hire clinically superb Full Time, Part Time and Per Diem RN Supervisors for the 3pm to 11pm shift. The RN Supervisors will oversee the day-to-day clinical activities of health care personnel, such as LPNs and CNAs. The RN Supervisor must be able to interpret patient information and make critical decisions about necessary actions, as well as administer medication, wound care and other personalized interventions.

Job Duties / Responsibilities:

• Completes admissions assessments and reassessment on all patients.
• Responsible for providing leadership and supervision to the nursing staff, housekeeping, and security.
• Works closely with medical support staff to ensure resident needs are met.
• Will greet admitted patients and prepare them for physical examination and treatment.
• Will maintain medical supplies and equipment.
• Provides education as indicated after assessing the learning needs of the patient, staff members.
• Assess the patient’s physical and psycho-social condition on an ongoing basis throughout the continuum of care.
• Utilize manual and automated information systems to improve efficiency and effect productivity. Accurately records patient information in the electronic medical records system and patient medical record.
• Demonstrate a current working knowledge of equipment, instrumentation and medications.
• Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals.
• Participate in preparation and facilitation of regulatory agency site visits to include DOH.
• In accordance with agency protocol and DOH standards, maintains adequate infection control standards.
• Reviews and reports all medical, accident/incidents related to patient care and provides input into actions necessary to resolve problems.
• Participate in weekly and on an “as needed basis” case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns.
• Ensure that personnel adhere to established policies and procedures.
• Document changes in resident's conditions.
• Participate in care planning meetings.
• Mentoring and supervision of LPN's and CNA's.

Requirements:

• Current NYS Registered Nurse license.
• 3 Years RN experience preferred.
• Current BLS Certificate.
