Job summary

Meadow Park Rehabilitation & Health Care Center in New York, NY is seeking an RN Supervisor to lead the subacute unit and oversee resident care using the nursing process.

You will coordinate with physicians and the interdisciplinary team, monitor residents, and supervise nursing staff while ensuring compliance with care protocols and regulatory standards. This role requires NY RN license and LTC supervisory experience.

Qualifications

• Licensed as a Registered Nurse in the State of NY.
• Past experience as an RN Supervisor in a Long Term Care facility.
• Knowledge of EMR systems.
• BCLS, ACLS or PALS certification.
• Excellent communication skills.

Responsibilities

• Managing the individualized resident care by promoting and monitoring residents' health through the nursing process.
• Utilizes clinical resources in implementing patient care and provides rationale for patient care decisions.
• Collaborating with physicians and interdisciplinary team members to resolve residents' problems and needs.
• Providing physical and psychological support to patients and their family members.
• Monitoring residents' conditions and administering medication & treatments.
• Supervising assigned team members.
• Completes Frequent Observations and the Intake/Outtake Record according to established protocols.
• Performs related duties, as required.
