Job summary

Civita Health in Milford, CT is seeking an RN Nursing Supervisor to lead nursing staff and ensure delivery of high-quality, patient-centered care across our facility.

In this role you will coordinate care plans with residents and interdisciplinary teams, supervise daily operations, monitor outcomes, and support discharge planning. This per diem position requires an active CT RN license, CPR certification, and relevant nursing experience; proof of COVID vaccination is required.

Qualifications

• Must hold a valid unencumbered RN license in the state of employment.
• Current CPR certification.
• Basic computer skills.
• Preferred: pain management and IV skills.
• Minimum 1 year of Skilled Nursing experience with Charge Nurse/Unit Manager/Supervisor roles.
• Proof of COVID vaccination is required.

Responsibilities

• Deliver high-quality, outcome-focused nursing care.
• Collaborate with residents and interdisciplinary care teams to develop individualized plans of care.
• Implement the plan of care consistently and cost-efficiently.
• Counsel residents/families and manage discharge planning resources.
• Share relevant information with physicians to update the plan of care.

• Collaborating with residents, responsible parties, and interdisciplinary care teams to develop individualized plans of care
• Implementing the plan of care consistently and cost-efficiently, with a focus on resident-centered outcomes
• Utilizing all appropriate resources for resident/family/responsible party counseling and discharge planning
• Collaborating with physicians and other healthcare professionals to share relevant information for changing the plan of care

Job Requirements:

To be considered for this position, you should meet the following requirements:

• Possession of a valid unencumbered license as a Registered Nurse within the state of employment
• Current CPR certification
• Basic computer skills
• Preferred: experience with providing pain management and IV skills
• Minimum 1 years of experience in Skilled Nursing with Charge Nurse, Unit Manager, or Supervisor experience
