Supervisor: Director of Nursing

Supervises: Neighborhood R.N., L.P.N., and Caregivers

Position Summary:

The Neighborhood Clinical Coordinator provides clinical leadership to the neighborhood, assuring each team’s abilities to promote the psychosocial well-being of all residents by coordinating, delegating and providing direct resident care. The Neighborhood Clinical Coordinator is responsible for supervising all neighborhood staff and leading the health care team members who provide direct resident care. Participates in planning, organizing, directing and facilitating the clinical operations of the neighborhood to ensure regulatory compliance, nurture a person-centered and resident directed culture, and promote individual and team learning. The NCC works cooperatively and respectfully with residents, coworkers, families, visitors and volunteers to create a positive and pleasant environment in the Neighborhood. This will involve direct problem solving, coaching and mentoring, maintaining appropriate documentation, conflict resolution and ensuring that the psychosocial and activity needs and desires of residents are met. Essential Duties and Responsibilities:

  • Provide knowledge, skills, and leadership to staff in all neighborhoods in specific areas of clinical accountability, assuring that the team has the needed resources (community links, materials, supplies, skills and knowledge) to be successful in the Nurse’s professional area; (specific areas of clinical accountability will be broad in scope for the Neighborhood Clinical Coordinator’s assigned neighborhood, and narrow in scope when serving all neighborhoods as mentor for a specific clinical areas of accountability such as infection control, wound management, education, admissions, and quality assurance.)
  • Assure the hiring, orientation, training, skills checks and evaluation of all staff in the assigned neighborhood is adequate to support quality outcomes. Guides neighborhood clinical staff to assure proper hiring, orientation, training, clinical competency, coaching, educating, counseling and performance appraisals are maintained in a manner supportive of team and professional standards of practice.
  • Guides the implementation of clinical policies and procedures which ensure safe, sanitary and efficient practices supporting professional nursing care standard; assures that documentation, medication administration, resident nursing care, safety and sanitation, personal and staff development, care plans, materials management, and resident rights are in compliance with regulatory and facility standards.
  • Assure training and evaluation of all staff is adequate to support quality outcomes in the in specific areas of clinical accountability.
  • Assure regulatory compliance by encouraging appropriate record keeping and charting in all neighborhoods and by maintaining appropriate files to document compliance in specific areas of clinical accountability.
  • Ensure all neighborhoods participate in Quality Improvement measurements that will surface questions and issues to address and that appropriate follow up is taken to assure quality outcomes, performing professional audits and observations to confirm the competency of all staff in the Neighborhood Clinical Coordinator’s professional area specific areas of clinical accountability.
  • Guides the neighborhood clinical team too efficiently and effectively use all resources, including maintaining clinical staffing and supply budgets within planned expenditures, maintain staffing patterns consistent with the philosophy, objectives and availability of resources.
  • Guides the clinical component of continuous quality improvement in the household in support of a systemic approach to quality clinical care.
  • Promotes the resident’s well-being and satisfaction by supporting resident centered care, developing and interpreting standards of care, guiding the interdisciplinary team as it establishes and implements the resident’s plan of care with the resident and his/her family or responsible party and the neighborhood team.
  • Maintain a safe, comfortable and functional environment by assessing the neighborhood for potential hazards, implementing practices utilizing appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness and emergency care, attending or assuring the household is represented on all appropriate councils, investigating and reporting all incidents and accidents, observing the team for safe work habits and addressing observations.
  • provide leadership and vision to the household by serving as a role model for core competencies, maintaining professionalism, supporting a learning environment by keeping abreast of latest trends and new ideas, collaborating with other household coordinators, mentors and guides to share resources, knowledge, expertise, opportunities and achievements

Duties:

  • Assist with MDS assessments
  • Care plan development and implementation process: annual, quarterly and with significant change
  • Conduct care plan meetings with residents and families on scheduled and as needed basis
  • Daily rounds for evaluation of resident quality of care and life and job performance of the Nurses and CNAs
  • Daily reporting of problems and resident changes in the morning staff meeting
  • Infection control, wound, safety surveillance and reporting for the neighborhood
  • Pharmacy system, med and treatment administration monitoring, MAR prep
  • MD rounds, prep and follow up
  • Documentation compliance for Nurses and CNAs
  • Incident review and follow up
  • Lab system monitoring and follow up
  • Admission, discharge and transfers process
  • Staff education, orientation, evaluation, household meeting coordination and conflict resolution

Education and/or Experience:

  • At least three to five years of work experience in supervision and administration required.
  • Prefer have experience working in Skilled Nursing, long term care, and CCRC environment.
  • Prefer having nursing home admissions, discharges and transfers experience.
  • Prefer experience using Point Click Care (PCC).

Certificates, licenses, registrations:

• Must have valid North Carolina RN License
