Summary

The Nurse Navigator is the liaison for patients in need of multiple services and support. The nurse works with all members of the department Team to seamlessly coordinate the care delivered, and provides the necessary support for patients and their families.


Qualifications

Education/Qualification
● BSN, MSN Preferred/RNP
● SCRN Certification preferred.
● Advanced Cardiac Life Support Certification, or obtain within one year of employment.
Three (3) years clinical experience in acute care. Experience in a protocol/pathway driven environment; able to deviate where appropriate and make referrals where warranted.

● Effective interpersonal skills with emphasis on customer service focused care delivery
● Ability to maintain confidentiality
● Ability to adapt to multiple and changing priorities
● Must be skillful in problem solving and being proactive in anticipating patient needs, taking appropriate course of action, using approved guidelines, protocols and pathways
● Work efficiently with diverse patient populations and diverse faculty in radiology, anesthesiology, physical medicine and rehabilitation, neurology, orthopedics, various referring physicians/practices sources, outpatient rehabilitation, clinical psychology, dietary and various adjunct multi-disciplinary service providers
● Proficient in Word, Excel and other computer operations
● Access information from EHR or outside hospitals/clinics
