Job description

Over 185 MESD employees work in School Health Services. We are committed to ensuring the safety of students by providing the highest standard of nursing care in the school and community setting so that every student has the opportunity to achieve their educational potential. We are nurses, care coordinators, health educators, health assistants, administrative staff, screening and prevention specialists who accomplish this by providing and coordinating specialized health services that support the educational experience for students throughout Multnomah and Clackamas County. Position:RN Specialist for Students with Complex Chronic Health Problems

POSITION PURPOSE :

A Complex Needs Nurse (CNN) provides nursing case management for students with complex health needs, which facilitates student placement and educational opportunities. The CNN also provides leadership by improving, supporting, and enhancing nursing practice within MESD schools and component districts. The CNN promotes student health and safety to improve school attendance and enhance the academic success of students.

NATURE & SCOPE :

This position is subject to board policies, administrative rules and procedures, department regulations, and applicable state and federal statutes. This position carries no administrative authority or responsibility.

ESSENTIAL JOB FUNCTIONS:

  • Be a leader in the department and set an example of expertise and professionalism.
  • Consult with nursing colleagues and the greater medical community.
  • Research emerging practices.
  • Develop and update nursing procedures.
  • Assess the health needs of referred students.
  • Obtain a health history from medical records and student/parent interviews.
  • Evaluate the student’s current health status through physical assessment and student/parent interview.
  • Obtain current medical information and orders from the student’s health care provider.
  • Facilitate school health services for identified students.
  • Develop a Student Health Management Plan (SHMP) with recommendations for the individual student’s health and safety based upon the nursing assessment.
  • Identify and re-evaluate changes in students’ health conditions; consult with school staff, parents, and health care providers to modify the SHMP.
  • Act as liaison between parents/guardians, school district staff, community agencies, and health care providers to ensure a safe and minimally restrictive school environment for students with complex health needs.
  • Participate as a member of the multidisciplinary team and make appropriate recommendations concerning student health/educational plans for Special Education and 504 services.
  • Maintain professional practice competencies.
  • Contribute to a positive and productive work environment.
  • Participate in designated meetings as directed by MESD administration.
  • Document nursing services provided, in accordance with agency standards and the Oregon Nurse Practice Act.
  • OTHER JOB FUNCTIONS:

    • Participate in kindergarten transition/IEP meeting to provide recommendations for nursing services that are needed for the student to attend school in a safe and healthy environment.
    • Perform nursing assessments as deemed necessary by MESD administration.
    • Provide backup support to the Nurse Consultant, by responding to body fluid exposure calls.
    • Provide consultation to nurses and school staff in component district schools.
    • Submit necessary documentation to allow districts to bill Medicaid for nursing services.
    • Perform other job-related duties as assigned by the Coordinator/Nursing Supervisor.

    KNOWLEDGE, SKILLS AND ABILITIES:

    • Willingness to work a flexible schedule.
    • Demonstrate computer fluency.
    • Demonstrate effective communication, leadership, and time management skills.
    • Valid driver’s license and vehicle available for use.

    EDUCATION AND EXPERIENCE:

    • Valid Oregon license to practice as a Registered Nurse.
    • Minimum of two years experience within the last four years working in a school/clinical setting with children who have chronic or technology-supported health problems.
    • Current First Aid/CPR/AED certification.
    • Knowledge of Multnomah County community resources is preferred.
