Job summary

vTech Solution is seeking a Registered Nurse (RN) to provide skilled nursing care in a Long-Term Care/Skilled Nursing Facility setting. The RN will use the nursing process and critical thinking to assess, plan, intervene, and evaluate patient care in collaboration with an interdisciplinary team.

Responsibilities include medication administration, patient education, documentation, and participating in performance improvement activities.

Qualifications

• Current Illinois RN license and CPR/BLS certification.
• Graduation from an accredited nursing program with education verification.
• Minimum of 2 years nursing experience in hospital, home health, or hospice care.

Responsibilities

• Perform patient assessments, develop and execute individualized care plans.
• Administer prescribed medications and treatments according to nursing standards and state laws.
• Document patient care accurately and comprehensively following system and entity standards.
• Collaborate with interdisciplinary team members to provide comprehensive patient care.
• Provide patient and family education tailored to their understanding and needs.
• Participate in department/unit performance improvement and shared governance activities.
• Provide clinical expertise and guidance to other units/departments as needed.
• Identify and resolve system breakdowns, escalating issues through appropriate channels.
• Manage complex patient care situations and delegate tasks appropriately.
• Engage in personal development and support continuous learning and evidence-based practice.
• Participate in peer review and feedback processes.
• Maintain timely patient flow and respond to telephone triage when applicable.

Scheduling:
