Now offering a $10,000 - $15,000 Sign-On Bonus depending on Experience

As a Home Health RN Case Manager, You Will:

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Confer with physicians to develop the initial plan of treatment based on orders and patient assessment. Provide hands-on care, management, and evaluation of the care plan, along with patient teaching, under supervision. Revise the plan in consultation with the physician as needed.
• Coordinate appropriate care with various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
• Report patient care, condition, and progress to the physician and Clinical Manager regularly.
• Implement the patient care plan in collaboration with the patient and family to achieve optimal outcomes.
• Supervise and coordinate the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, including providing written care instructions and monitoring documentation completion.
• Discharge patients after consultation with the physician and Clinical Manager, completing all necessary documentation.
• Prepare medical documentation including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan modifications. Contribute to visit reports and patient records, and collaborate with other healthcare disciplines for optimal care.

Required Experience/Skills:

• Diploma, Associate, or Bachelor Degree in Nursing
• Minimum of one year of nursing experience in skilled Home Health
• Strong med-surg, ICU, ER, or acute care experience (if no prior Home Health experience)
• Experience with OASIS & HomeCare HomeBase
• Current and unrestricted RN licensure
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

Pay Range:

• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 per year base pay

Scheduled Weekly Hours: 40
