Qualifications

Responsibilities

• Implements and monitors patient care plans and communicates patient condition.
• Serves as primary coordinator of care across disciplines.
• Follows physician and nursing orders precisely.
• Coordinates discharge planning with the healthcare team.
• Performs comprehensive clinical assessments and ongoing re‑assessments.
• Develops daily goals and a plan of care with patient/family.
• Delivers nursing care and documents in the health record.
• Evaluates patient status and adjusts care plans as needed.
