Job summary

Centene is seeking an RN Senior Transition Specialist to lead care coordination and transitions in the community. You will develop transition plans with the care team, educate members and families, and ensure authorizations are honored throughout the transition process.

Ideal candidates have 4–6 years of related experience, a Bachelor's degree, and RN licensure preferred for NH Health Families. This is a remote role based in New Hampshire with day shift, full-time scheduling.

Responsibilities

• Work with care coordination team, providers, and other healthcare team members to develop effective transition plans.
• Assist with the transition of care for members based on enrollment or identified services.
• Identify new member enrollees requiring transition services.
• Ensure existing authorizations are honored during the transition process.
• Educate members and their families on services, requirements, limitations, and exclusions.
• Review transition metrics to identify trends and process improvements.
• Partner with leadership to identify process improvements for transition of care.
• Assist with developing educational materials and resources for members.
• Develop training programs for care coordination staff and providers.
• Evaluate member needs and recommend plans for optimal outcomes.
