What You Will Do

To improve the quality of life of patients and families by coordinating patient care processes of assessment, planning, implementation and evaluation in collaboration with the patient, family, physician, and appropriate support services for the age population served, as defined in the department's scope of service.

  • Conduct thorough physical and psychosocial assessments, including functional and nutritional screenings.
  • Use assessment data to determine nursing diagnoses and develop prioritized care outcomes.
  • Collaborate with interdisciplinary teams to establish individualized plans of care, updated daily or as patient condition changes.
  • Deliver direct patient care, administer treatments and medications, and provide patient/family education.
  • Monitor patient progress through systematic reassessments and adjust care plans accordingly.
  • Perform triage assessments in accordance with departmental protocols.
  • Demonstrate proficiency in managing patients across all age groups, including those receiving Procedural Sedation
  • Maintain comprehensive documentation of care activities and patient education.
  • Uphold patient rights and safety standards at all times.
  • Perform other duties as assigned
