Road to Responsibility (RTR) is a nonprofit organization dedicated to providing individuals with disabilities the opportunity to live fulfilling lives within their communities. We are currently seeking an RN to join our team of nurses in providing clinical oversight and compassionate care for individuals residing in our residential homes.
In this role, you will:
- Collaborate with a team of nurses to ensure high-quality care and services.
- Support and educate our non-clinical staff through regular training.
- Coordinate medical services and care plans for individuals in our programs.
- Serve as a key liaison between RTR and the Massachusetts Department of Developmental Services (DDS).
- Prior experience in human services is a plus but not required—we welcome RNs from all backgrounds who are ready to make a difference.
Job Description:
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Provide clinical consultation regarding complex healthcare issues or newly diagnosed conditions that require an in-depth healthcare management plan.
- Review Physician Orders, including treatments, protocols, and medications with the Residential Manager and/or Director.
- Review clinical records on a regular basis to ensure adequate services and support are provided, referrals are obtained as needed, health screenings and diagnostic tests are done, as needed.
- Periodically review the health status of individuals by observing and interviewing individuals, examining documents and making recommendations.
- Follow hospital admissions and act as a liaison between RTR and DDS for all medical information.
- Assure that appropriate resources are available, staff are trained to provide follow-up care after an emergency or health episode, e.g., VNA services, hospice, etc.
- Assist RTR Management with admission and discharge planning.
- Review DDS’ Health Screen Recommendations with the Managers and/or Directors to provide all individuals with healthcare services in accordance with DDS regulations.
- Attend DDS meetings and/or other required meetings concerning healthcare planning.
- Complete the Department of Public Health’s approved “MAP Train the Trainer” program.
- Review MOR’s in assigned homes/programs at least quarterly.
- Visit assigned homes regularly to determine compliance with MAP. Conduct audits using the MAP Technical Assistance Tool periodically.
- Attend team meetings concerning individuals’ healthcare/medical issues.
- Provide communication concerning medical issues to all ISP team members as requested. Establish and maintain strong communication links with managerial staff.
- Interview LPN job applicants and make hiring recommendations, as requested.
- Provide staff training as necessary. Provide clinical supervision for LPN staff.
- Participate in on-the-job training and coaching for new co-workers as directed.
- Adhere to agency safety standards.
- Perform additional duties or substitute for others as required. Follow all RTR policies and procedures.
- Develop and provide all services in accordance with RTR's philosophy and mission.
Qualifications:
- Registered Nurse diploma or bachelor’s degree with three years related experience and/or training in a community health setting.
- Valid Registered Nurse license.
- Valid driver’s license.
- Basic knowledge of Microsoft Word and the internet.
- Successful completion of all state and agency mandated training.
- Willing to authorize a Criminal History Background Check (CORI) and release of information concerning DDS, DPPC, and other agencies.
- Ability to perform each essential duty satisfactorily. Reasonable accommodations may be made for individuals with disabilities.
Language Skills:
Must be fluent in English and able to read and interpret safety rules, operating and maintenance instructions, and procedure manuals. Capable of writing routine reports and correspondence. Able to communicate effectively with individuals under supervision, employees, and clients.
Physical Demands:
Occasional standing, walking, sitting, handling, reaching, climbing, stooping, kneeling, crouching, crawling, talking, and hearing. Occasionally tasting or smelling. Frequently lifting/moving up to 25 pounds and occasionally up to 60 pounds. Specific vision abilities required include close, distance, color, peripheral, depth perception, and focus adjustment.
Reasoning Ability:
Ability to carry out detailed written or oral instructions.
