Job summary

UofL Health is seeking an Admission Coordinator at Frazier Rehab Institute - UMC in Louisville. You will work with referring facilities to identify suitable rehab candidates, review medical records for necessity, and coordinate transitions from acute care to rehab, ensuring patient needs and precerts are arranged.

You will educate patients and families, coordinate with payers, and help maximize referrals while maintaining department operations aligned with the hospital's mission and standards.

Qualifications

• Kentucky RN license (Associate or Bachelors Degree) or other licensed clinical professional is required.
• Five years minimum of nursing/licensed clinical experience working with a broad base of patient types/diagnosis.
• Previous rehabilitation experience required and knowledge of Medicare, Medicaid, and third party payer systems.
• Certification: Basic Life Saving (BLS) accreditation by the AHA is required.

Responsibilities

• Assist in delivering high quality health care services in a professional, compassionate, and courteous manner.
• Establish and maintain referral sources with staff at referring facilities.
• Evaluate medical records to identify rehab potential according to Medical Staff criteria.
• Educate and prepare the patient and/or family for admission.
• Explore and secure financial coverage for services.
• Plan and organize department operations within the hospital’s strategic plan.
