Job summary

UofL Health is seeking an Admission Coordinator for the Frazier Rehab Institute in Louisville to manage referrals and preadmission assessments. You will liaise with referring facilities, physicians, payers, and families to ensure smooth transitions from acute care to rehab and coordinate precerts and insurance verification.

The successful candidate will have Kentucky RN licensure, at least five years of nursing/clinical experience with rehab exposure, and strong communication and organizational

Qualifications

• Kentucky RN license (Associate or Bachelors) or other licensed clinical professional.
• Five years minimum nursing/licensed clinical experience; rehab experience preferred.
• Knowledge of Medicare, Medicaid and third-party payer systems required.
• Preferred: marketing/sales background and patient/family counseling experience.

Responsibilities

• Collaborates with referring facilities to identify potential rehab patients.
• Evaluates medical records to determine rehab potential per Medical Staff criteria.
• Presents patient condition to hospital staff accurately for admission.
• Educates patient/family for admission and prepares them for transition.
• Secures financial coverage and handles precerts with insurance verification.
• Plans department programs; ensures efficient operations and referrals.
• Maintains compliance with policies and standards; performs other duties.
