Job summary

Yale New Haven Health is seeking an Outpatient Wound And Skin Resource Nurse to provide clinical oversight for wound care within the Center. You will oversee patient care from assessment through evaluation for all ages, ensuring adherence to safety and infection control standards.

Requirements include a BSN (or progress toward within three years), 3+ years RN experience with at least 1 year in wound care, CT RN license, and BLS. Hyperbaric and outpatient experience are preferred.

Qualifications

• Graduate of an approved school of nursing.
• BSN required within three years of assuming position.
• Minimum of three (3) years nursing experience, with one (1) year of dedicated wound care experience.
• Wound care certification required, or within one year of hire.
• Must have a current State of Connecticut nursing license.
• BLS required.
• Hyperbaric experience preferred.
• Outpatient experience preferred.

Responsibilities

• Coordinates entire delivery of patient care both in and outside of center.
• Plans, implements, and evaluates educational activities to meet health needs of patients.
• Evaluates equipment and makes recommendations for wound care management.
• Keeps current with technology and products used in wound care.
• Coordinates patient care with inpatient wound and skin care nurse.
• Meets performance expectations for customer service and teamwork.
• Maintains confidentiality of hospital, staff and patient information.
• Documents in the electronic medical record.
• Participates in patient and family teaching.
• Maintains infection control standards.
• Performs other duties as assigned to ensure smooth operation.

Qualifications
