Job summary

Yale NewHaven Health is seeking an Outpatient Wound and Skin Resource Nurse to provide clinical oversight for wound care within the Center. Responsible for patient care across all ages, from assessment to evaluation, in alignment with our mission and values.

Qualifications include a BSN within three years of hire, a minimum of three years nursing experience with one year in wound care, and CT nursing licensure with BLS. Hyperbaric experience and outpatient wound care exposure are preferred.

Qualifications

• Graduate of an approved nursing program; BSN required within three years of starting.
• Minimum three years of nursing experience with at least one year in wound care.
• Current Connecticut nursing license and BLS certification.
• Wound care certification either current or obtained within one year of hire.

Responsibilities

• Coordinate overall patient care inside and outside the center.
• Plan, implement, and evaluate educational activities to meet health needs.
• Assess equipment needs and make recommendations for wound care devices.
• Stay current on wound care products and technologies.
• Ensure compliance with applicable regulations and standards.
• Collaborate with inpatient wound and skin care teams.
• Meet performance expectations for service, teamwork, and development.
• Maintain confidentiality of hospital, staff, and patient information.
• Document in the electronic medical record.
• Participate in patient and family teaching.
• Maintain infection control standards and safe environment.
• Perform other duties to ensure smooth department operations.

• 1.Coordinates Entire Delivery Of Patient Care Both In And Outside Of Center.
• 2.Responsible For Planning, Implementing, And Evaluating Formal And Informal Educational Activities To Meet Identified Health Needs Of The Patient.
• 3.Demonstrates Knowledge Of Available Equipment And Selects Appropriately For Individual Patient Needs; Evaluates And Makes Recommendations For Changes In Equipment And Related Products Utilized In Wound And Skin Care Management.

• 5.Familiar With Applicable Dph Regulations.

Qualifications

EDUCATION

Graduate Of An Approved School Of Nursing; Bsn Required Within Three Years Of Assuming Position.

EXPERIENCE

Minimum Of Three (3) Years Nursing Experience, With One (1) Year Of Dedicated Wound Care Experience Required. Hyperbaric Experience Preferred. Outpatient Experience Preferred. Wound Care Certification Required, Or Within One Year Of Hire.

LICENSURE

Must Have A Current State Of Connecticut Nursing License. BLS Required.
