• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Documents steps of the nursing process and patient data accurately.
• Collaborates with multidisciplinary team and facilitates appropriate referrals and discharge planning.
• Ensures patient safety and adherence to unit policies and procedures.

Addresses problems and progress toward goals. Facilitates patient discharge through timely, appropriate referrals and utilization of patient education resources, suitable to patient age and cultural/psychosocial factors. Collaborates with other disciplines as appropriate. Supports unit/hospital goals through PI and educational activities as evidenced by attending in-services, meeting unit education standards, and completing variance reports.Decision Making: Independent judgment in making decisions from many diversified alternatives that are subject to general review in final stages only.Working Relationship: Coordinates activities of others (does not supervise).
