Allay Hospice is seeking an On-Call Registered Nurse to provide hands-on hospice care during after-hours across the territory. The role involves 7 on, 7 off, 4:30 PM–8:00 AM Monday–Friday and alternating weekends, with travel to patient homes and facilities.

The RN will assess symptoms, administer medications per orders, and collaborate with the physician while supporting families through transitions. Benefits include health coverage, 401(k), PTO, and mileage reimbursement.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsabilités

• Provide 1:1 care to patients and families during after-hours.
• Collaborate with Remote Triage On Call Team to deliver hands-on hospice care.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols.
• Communicate changes in condition to the physician and obtain orders.
• Pronounce death and support family if death occurs during on-call hours.
• Complete timely documentation and communication.

Office Location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045

Coverage area: Travel required throughout the Allay Hospice Territories

Schedule: 7 ON 7 OFF 4:30PM-8AM Monday-Friday and every other weekend Saturday 8AM-Monday 8AM

As a On-Call RN You will:

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
