Job summary

Adoration Home Health is seeking a Registered Nurse to join our Leesburg-based team for PRN home health visits. You will provide patient-centered, 1:1 care in patients' homes across Loudoun County.

Responsibilities include planning and delivering MD-ordered care, administering meds, and educating families. Valid RN license and CPR required; driving is necessary.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted license in the state of practice and residence.
• One year nursing practice in patient care; home health or geriatrics preferred.
• Valid driver's license, driving record, and car insurance per Adoration policy.
• New graduates may be considered in select markets based on program availability.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligning with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on‑call rotation as required by the local branch.
