Job summary

Phoenix Home Care & Hospice in Liberty, MO is seeking an experienced RN for full-time home health. You will manage patient caseloads, supervise LPN visits, and ensure high-quality, compliant home-based care.

You will collaborate with physicians and an integrated therapy team to implement diagnosis-driven plans and educate families on conditions like CHF, COPD, diabetes, wounds, and medications. We value compassion, reliability, and strong clinical judgment.

Qualifications

• Active Missouri or Compact RN license is required.
• Home health experience strongly preferred.
• Strong clinical skills in physical assessment and diagnostic interpretation.
• Compassionate communicator with patients and families.

Responsibilities

• Manage your own caseload including supervision of LPN visits.
• Varied weekly visits including routine care, evaluations, and starts of care.
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care.
• Work alongside a strong therapy team including PT, OT, and Speech.
